PRIORITY: Physical Therapy (PT) Services

DESCRIPTION: The Sevier School District Department of Special Education located at 180 E. 600 N. Richfield, UT 84701 is soliciting sealed bids for physical therapy services from qualified firms/individuals for the Special Education Department of Sevier School District

PROPOSAL DEADLINE: August 14, 2017 at noon.

SUBMISSION: The sealed bid for the Physical Therapist Proposal will be received by the Sevier School District ATTN: Mrs. Lisa Crane, Director of Special Education at the

Sevier School District Office Building located at 180 East 600 North, Richfield, UT

EVALUATION OF PROPOSALS: A committee will review and rank the proposals based on various factors including but not limited to company history, qualifications (to include person specific information regarding licensure, degrees, etc. of the personnel who will be providing the service), experience, references, program design and implementation plan, and price. Proposals submitted without specific personnel license information will not be considered. The committee will enter into negotiations with the individual or company that receives the number one ranking. The notice of intended decision will be mailed to all bidders if requested.

QUALIFICATION CRITERIA: The primary interest of Sevier School District is to secure the highest quality services at a reasonable cost. The provider must demonstrate the capability to perform the physical therapy and possess the integrity and general reputation of a reliable firm. The individual firm should be able to handle all anticipated matters involving a public school system with a general population of more than 4,500 students.

TERMS: The District reserves the right to reject any and all proposals. The District may enter into negotiations with one or more of the applicants following the review of initial proposals, which negations may result in the modification of terms and conditions in original proposals.
